Department Function:
Supervise and implement care coordination and community health initiatives.
Summary of position:
Registered nurse tasked with care coordination for individual patients as well as the implementation of community health initiatives.
Supervise Care Coordinator and Community Health Educator.

Essential Duties:
Care Coordination:

  • Work with Care Coordinator to complete patient health risk stratification.
  • Plan and coordinate care, resources and services on a case-by-case basis with family members, medical professionals, and other health service providers.
  • Accurately document and submit medical records.
  • Demonstrate knowledge in medical care, such as diagnostic procedures, medication, symptoms, and other treatment-related therapies.
  • Provide culturally competent support to patients and families.
  • Create an environment for open communication between the patient, Care Coordinator, Physician and other clinic staff( MA’s, Patient Care Assistants).
  • Act as a patient advocate to ensure that individual needs are met
Community Health:
  • Work with Community Health Educator to implement community health initiatives.
  • Provide culturally competent health education.
  • Provide clinical oversight of clinical outreach and monitoring programs, beginning with the Remote Blood Pressure Monitoring Program.
 Minimum Qualifications
  • Bachelor’s Degree in Nursing
  • Experience with electronic medical records system preferred
  • Bilingual in Asian language or Spanish preferred.
Additional Qualifications:
  • Experience with general office software (Microsoft Word, spreadsheets and EMR)
  • Experience with Athena and Dentrix Ascend systems preferred
  • Provide services in courteous and pleasant manner
  • Ability to put the patient at ease, make the patients visit smooth
  • Knowledge of OSHA specific infection control guidelines

Physical Requirements and Working Environment:
  • Frequent walking, standing, bending. Frequently work in a seated position at a desk.
  • Respond and react immediately to verbal instructions/requests and to auditory signals from equipment and personnel.
  • Physically close and distance visual activities involving objects, persons, and paperwork, as well as discriminate depth and color perception.
  • Communicate effectively, both orally and in writing, using appropriate grammar, vocabulary, word usage, and interpersonal skills.
